- Bachelor of Physical Education (B.P. ED) Marksheet (Final Semester) (JPG / JPEG File format)
- Only highest Sports Achievement / Participation Certificate (JPG / JPEG File format)
- Caste / Reservation Certificate, if any (JPG / JPEG File format)
